On the form: Name and Email fields should be name and email address of staff submitting the request
Provide Califa with the following information in the Message field:
The old card number
The new card number
The patron's name
The attraction name (if possible)
Indicate whether the patron just needs the reservation transferred, or they need a printed pass (as a PDF) too. (A printed pass should only be necessary if their reservation is fairly soon, like today or tomorrow.)
